Any time you’d like to make a new issue available to users of your app—whether free or paid—you will create it in the Issues tab on your Publish site and upload your issue mib file.
The first time you go to the Issues tab, your only option will be to add a new issue. Then after upload it will be listed under Unpublished, and when it is published (that is available to your app readers) it will be listed under published.
Click Add New Issue.

For all app versions from 3.0 the 2 following images are not needed.

Check the box for the platform(s) you want the issue to be available on.

To create the issue press create.
Now you will be presented with information about your issue and also the option to upload your MIB for that issue.If you don’t have the MIB file ready now you can upload it later. Also all necessary assets that aren't updated will be marked red and need to be uploaded before the issue can be published.
To upload a MIB, click the Upload link. If you wish to host the MIB from your own server, select 'enter a URL instead' and enter the full URL pointing to the MIB file. Otherwise just click 'Browse', select your MIB and it will be hosted with our servers. Select if it is a Retina optimized or Standard MIB. Finally click Upload.

All devices that are defined as test devices in publish, will be able to see the unpublished issues in the Library of the app.
When you are satisfied that your issue is finished and all the lights are green click Edit.

At the bottom of the issue page press 'Save and Publish'.
